A number of changes to switch the code from LGPL-2.1 to BSD-3:
...
- Updated LICENSE, README, CHANGELOG and CONTRIBUTING.md
- Removed COPYRIGHT, added NOTICE
- Updated the headers in all source files, makefiles, etc.

Add three more options for MFEM_TIMER_TYPE
...
* 4 - an improved option for Mac (the new default for Mac),
* 5 - version based on gettimeofday, and
* 6 - version using MPI_Wtime.
In makefile, add target 'print-*' that prints the contents of any
makefile variable, '*'.
